Injury / Incident Reporting Procedures

There are different reporting procedures depending on the type of incident that has occurred. Please see the appropriate category below for details: 


Report an employee or work-related injury:

1. Notify your supervisor immediately.  
2. Complete First Report of Injury (FROI) within 8-hours and forward to Workers Compensation. Click here to obtain FRI and submit electronically
3. Call EHSO as soon as possible and verbally report injuries involving medical attention so an investigation can be conducted.Andy = 218-726-6730

Report a non-employee, student, non-work related or property damage incident:

1. Notify your instructor, TA, or a university employee immediately.  
2. Complete a Bodily Injury / Property Damage Incident Report and forward to Risk Management. Bodily Injury / Property Damage Incident Form
3. Call EHSO as soon as possible and verbally report injuries involving medical attention so an investigation can be conducted.Andy = 218-726-6730

Report a biohazard-related injury (related to needle sticks, sharps, blood and body fluids, or anyone working under an IBC Protocol):

1.  Notify supervisor immediately. 
2.  Complete paperwork for work-related injury, above.Work Related Injuries.
3.  Non-employees need to complete paperwork for non-employee injury, above.Non-employee Injury.
3.  Report verbally to EHSO at 218-726-7273 or 218-726-7139 and seek medical attention.St. Luke's Occupational Medicine (218-249-6822), Essentia Health Occupational Medicine(218-786-3392), or Health Services (218-726-8155) are the preferred providers.
4.  Complete the Institutional Biosafety Committee Incident Report and forward to EHSO. This can also be done via e-protocol.IBC Incident Report
